Effective oversight requires three conditions at once. The human must be informed: the signal reaches them while action is still possible. Authorised: they hold the halt authority to act, not merely to advise. Timely: the act lands before the reversibility window closes. Remove any one and oversight collapses into observation. The person sees the failure and records it, but the outcome is already fixed.
Presence is not capability.
A human in the loop is often presented as the safeguard. It is not, by itself, evidence of anything. The intervention chain still has to complete: the human must detect the deviation, escalate or decide, and intervene in time. Effective human oversight is the name for the case where all of that holds. Intervention Readiness is how it is measured.
